Have you completed the registration, but have questions about forms?

There are 3 forms:
1. The Parental Consent and Medical Form – print and fill out (please attach copy of insurance card)
2. The Photo Permission Form – print and fill out

Both of these should be sent to:

Cary, NC 27512

3. The Physical – this form is ONLY for students NEW to the band program. New students should bring this form to pre-camp.
