WCPSS Forms
The following forms are required for marching band, winter guard, and winter percussion.
- WCPSS Parental Consent & Medical Form (digital fill & sign)
- WCPSS Student Photo Permission Form (digital fill & sign)
- WCPSS High School Athletic Participation Form (print & fill out; portion filled out by physician) (first year only, unless the student has major health changes)
- WCPSS Parent Request & Physician’s Order for Medication (Form 1702) (print & fill out; portion filled out by physician)
